How to get a referral

Referral agencies

Only accept named agency referrals via Hammersmith & Fulham PATHS Team.

Referral procedure

Phone Mon-Fri 9am-5pm or write. Application form completed by referral agency and applicant. Assessment and interview. Require background information and proof of ID. Must be eligible for benefits.
